Just run the D2ModSetup.bat in your Diablo II folder not d2elements folder. It's woking for me. :)

Have done exacly what the readme says.
Especialy in the rigt click properties you must have like this:

And have d2mod.dll in d2 folder and run d2modsetup.bat there.

Also you must have in the start-up screen(that with single player, other multiplayer etc.) in the lower left corner written like this: D2 Elements v1.01 and D2 Mod System v1.02; in the lower right corner plugy version.
Also must have v1.10.
Also refer to this topic(might help):

Hi together,

sorry for not being here, I just had to do some holidays :). Now I am back...
For your problem, it is most likely that this has something to do with the d2mod installation as cosma_sebastian says, as this is known to cause ITH entries for some strings. Can you check if you see all the version displays that cosma_sebastian is telling about, and tell me if any of them are missing?

yes, for sure :). the d2mod system does not run on v1.11, and thus my mod doesnt run on that version. I am sorry you have to switch back... there is a tutorial somewhere about how to get 2 versions of diablo2 running on one installation though :).
