This is assuming your router is Linksys and password is the default one which is admin. Perhaps someone could post for other types of routers?
Step 1) Check your IP in Diablo 2 or if you are using 95/98/(2000 im not too sure about) click Start>Run> winipcfg [enter] This is the IP you will need
(192.168.1.10#)
Step 2) Login to router. Linksys' address is 192.168.1.1, default password is admin ( no username )
Step 3) Click Advanced (yellow box) > (Port) Forwarding
Step 4) There should be five columns in this order in the white area:
Ext.Port // Protocol TCP // Protocol UDP // IP Address // Enable
Step 5) Enter this:
4000 to 4000 // Check // Check // 10# ( ip address gained above) // Check
Step 6) Next line:
6112 to 6119 // Check // Check // 10# // Check
Step 7) Hit Apply
You should be able to host now. Congratulations

While the process isn't exactly the same, opening up ports 4000, 6112 - 6119 should work for other routers as well. It works on mine at least.

Port 4000 is only needed if you want to host tcp/ip game.
Port 6112-6119 is used if you want to host on battle.net.
