question bout a weird unique charm

i found a unique small charm but the pic is of a jewel,i tried to id it and i cant.it says keel in inventory to get mods like a charm and looks like a jewel.this is single player if that means anything

well i figured out the elder thing,but he is hard lol.though i liked the cows.now baals minions was so longgggg then i got in there and well meph came out attacking and i killed him....then baal baal clone and Diablo came out i got clone dead and got baal down like || then i got unhandled exception X_X it took like hour to get to him.and i get unhandled exception when i try to go to meph everytime
