Try setting the output1 lvl field to 99 and put the name of the unique Sword in the output1.

For sounds look in MonStats.txt
MonSound & MonSound are the sound effects for a monster.

All unique items can be removed unless it is a quest related item, like with the runes.
The game will generate a rare item if there is no unique equivalent.
If the game code does not need it to do something it can be safely removed I would think.

As long as none of the Runewords have the Complete flag set then no runeword can be made.

Did you remove the runes from Itemtypes.txt?
Runes are also listed in Misc.txt file.
Gems.txt only is there to give gems and runes properties.

Not knowing how the Hell forge selects the runes to be dropped but I am going to guess it is going to use the different runes item codes.
If so then make all the runes into nonsocketable items that can be used in the Cube for whatever you wish.

That chance is for every Cow killed. It could happen on the first kill of the first run or maybe never...

Weapon.txt for the weapons base speed but what really affects speed is the animations for the different actions.

To drop specific set and unique items you can use the name of the item in TCEx file.
As Ogden stated and I am not totally sure but the monster level will have to be high enough to drop the item.
